On connect you authorize a Notion workspace (and pick a parent page if you granted several); forms.ax creates a database with your fields as properties. Every completed response becomes a new item — triage them with Notion views, statuses and assignments.

Under a page you grant access to. If you grant several pages, you pick the parent once during connect.
