Field types
Every form is built from these 12 primitives. There is deliberately no password or PIN type — forms.ax never collects credentials.
short_text A single-line text answer - names, titles, short free text. Supports AI follow-ups and semantic validation. · AI follow-ups long_text A multi-line text answer for descriptions and open feedback. The natural home for AI follow-ups. · AI follow-ups email An email address, format-validated on submit. Used for respondent-level erasure and HubSpot contact matching. phone A phone number with input hints and sanity validation. number A numeric answer with optional min/max bounds - quantities, budgets, ages. single_choice Pick exactly one of 2+ options - radio buttons. Options must be unique. multi_choice Pick any number of 2+ options - checkboxes. date A calendar date - deadlines, event dates, preferred appointment days. file_upload File uploads - images (JPEG/PNG/WebP/HEIC) and PDFs, streamed to EU storage. 10 MB/file Free, 25 MB paid. signature A drawn consent signature stored as an image with the response. rating An integer scale rating, 1-5 by default, any integer span up to 10 (e.g. NPS 0-10). url A web address, validated as a URL - portfolios, company sites, social profiles.
